I regularly use Quidco and find that Alamo or ebookers(again usually Alamo) are the cheapest and have had no problems.
Also netflights via Quidco seem to be competitive, they have dropped Thrifty/Dollar thank goodness and their cheapest offers again are Alamo
I usually rent a mid range SUV such as a Ford Escape or Jeep and can recommend the former

Any other strategies people are using? Particularly on the insurance front.

The insurance that comes with the UK issued Amex Platinum Card is sufficient (they state this clearly) that you decline all insurances offered by the US car hire company.

I have this set in my profile with both Hertz land National (my preferred company) and never had an issue.

As I hire a lot in the US I find the £450 annual fee, which also provides travel insurance for the good wife and kids as well, to be fair value.
