Name changes are prohibited by most airlines and always have been.

A number of no-frills carriers do allow limited changes, but largely as a means of generating another revenue stream.

The otherwise almost universal restrictions on name changes are designed to limit the chances of individuals or agents speculatively buying up all the cheapest tickets, and then subsequently re-selling them at higher prices to unsuspecting passengers.

If you are likely to need a name change then the airlines offer fully-flexible tickets which can be cancelled and then a replacement one bought in the new name.
